How Much Weight Can You Actually Lose on GLP-1s? (Realistic Timelines)

If you live in Chattanooga or Signal Mountain and you’re considering a GLP-1 medication for weight loss, you’ve probably seen the dramatic before-and-after photos. What you’ve likely not seen is an honest, physician-led explanation of what the numbers actually look like, how long it takes, and why results differ from person to person. Here’s the straight answer to the question we hear most: how much weight can you really lose?

What the Clinical Trials Actually Show

These medications are among the most studied weight-loss treatments ever brought to market. Here’s what the research found (these are group averages from clinical trials — individual results vary):

  • Semaglutide: about 14.9% mean body-weight reduction at 68 weeks (STEP 1, NEJM 2021). The newer higher-dose 7.2 mg formulation reached roughly 20.7% at 72 weeks (STEP UP).
  • Tirzepatide: about 20.9% at the 15 mg dose over 72 weeks (SURMOUNT-1, NEJM 2022).
  • Head-to-head: tirzepatide produced about 20.2% loss versus semaglutide’s 13.7% at 72 weeks (SURMOUNT-5, NEJM 2025).

To put that in real terms: a 250-pound person averaging 15% loss would be down roughly 37 pounds; at 20%, about 50 pounds. Again — these are averages, not promises.

Most People Respond, But Not Identically

In the semaglutide STEP 1 trial, 86% of participants lost at least 5% of their body weight, 69% lost at least 10%, and 50% lost at least 15%. That’s why a one-size-fits-all telehealth script doesn’t make sense — your body’s response is your own.

If you have type 2 diabetes, expect somewhat less: roughly 10% on semaglutide (STEP 2) and about 15% on tirzepatide (SURMOUNT-2). This blunting is well documented, and it’s exactly the kind of nuance a physician should set expectations around before you start.

The Realistic Timeline

Weight loss on a GLP-1 is a marathon, not a sprint:

  • Weeks 1–16: Dose is escalated gradually (typically over 16–20 weeks). This slow ramp is intentional — it minimizes side effects. You’ll see some early loss, but the dose isn’t yet at its full effect.
  • Months 4–12: This is where most of the loss accrues, month over month.
  • Around 12 months: Weight loss tends to plateau, and with continued therapy it’s maintained out to two years (STEP 5).

Anyone promising you 30 pounds in a month is selling hype, not medicine.
