Do You Qualify for GLP-1 Weight Loss? A Chattanooga Physician Explains

One of the most common questions I get at Summit Metabolic Health here in Chattanooga is simple: “Am I even a candidate for this?” People have read about semaglutide and tirzepatide, they’ve watched the success stories, and they want to know whether the medication is for them or only for someone heavier or sicker. Let me walk you through how physicians actually decide.

The Two Numbers That Start the Conversation

GLP-1 medications approved for chronic weight management follow a consistent eligibility framework built around body mass index (BMI):

  • BMI of 30 or higher — this is the obesity threshold, and on its own it generally makes you a candidate.
  • BMI of 27 or higher, plus at least one weight-related health condition — examples include type 2 diabetes, high blood pressure, high cholesterol, obstructive sleep apnea, or fatty liver disease.

BMI is a starting point, not the whole story. It doesn’t distinguish muscle from fat, and it can misclassify very muscular or very petite people. That’s exactly why we don’t treat a number on a chart — we treat a person sitting in front of us.

What “Weight-Related Condition” Really Means

You don’t need to be diabetic to qualify at a BMI of 27. Many patients I see have prediabetes, sleep apnea they didn’t even know was tied to their weight, or a blood pressure that’s crept up over the years. Part of a good evaluation is finding those connections, because they change both your eligibility and your urgency.

Who Is Not a Candidate — and Why We Screen

A responsible program screens you out, or pauses, when there’s a real reason to. We look carefully at:

  • Personal or family history of medullary thyroid cancer (MTC) or MEN 2 syndrome. These are contraindications to this class of medication.
  • History of pancreatitis — not an absolute bar, but something we weigh carefully.
  • Pregnancy, or plans to become pregnant. These medications are not used in pregnancy.
  • Certain GI conditions that delayed stomach emptying could worsen.

This is the difference between a physician-led clinic and a website that ships pens to anyone who clicks “yes.” The screening is the safety.

Age, Weight History, and “I’ve Tried Everything”
