Four things that are true here and almost nowhere else
Every large telehealth weight-loss company takes your money before a clinician looks at your chart. That is the model — volume, a prescriber pool, a support queue. Summit is one physician with a capped patient panel, so it can afford to work the other way around.
You talk to him before you pay anything.
The consult is free and no card is required to have it. Most programs take payment at intake or at signup, before any clinician has read a word of your chart.

A physician with a capped panel has no reason to enroll someone who shouldn’t be enrolled. That is the part a volume model cannot copy.
Why Florida is different, and why that matters to you
Florida has more direct-to-consumer weight-loss telehealth than anywhere else we serve. Some of it is good. A lot of it is a website, a questionnaire, and a prescriber you will never speak to, priced to look cheap until the third month.
The seasonal thing is real, and it is the reason a lot of Florida patients pick us specifically. If you winter in Naples and spend summers in Ohio or Tennessee, you do not have to change doctors twice a year — those are all states I am licensed in, and your chart, your dose, and your phone number stay exactly where they are.
Florida also has the most aggressive medspa market in the country, which means the most people arriving here mid-treatment, on a dose someone escalated too fast, with side effects nobody warned them about. That is a fixable situation and it is a normal way to start.

Serving all of Florida
Dr. Miranda holds an active Florida medical license and sees Florida patients by video from the practice’s home base outside Chattanooga, Tennessee. Visits are by video, so the whole state is in range — Miami, Orlando, Tampa, Jacksonville, St. Petersburg, Fort Lauderdale, Tallahassee, Naples, Sarasota, Gainesville, Cape Coral and Pensacola, and everywhere in between. Florida has more direct-to-consumer weight-loss telehealth than any state we serve, and most of it never puts you in front of a licensed physician. This one does — every visit here is a video visit with a Florida-licensed doctor, not a questionnaire scored by software. Florida telehealth is governed by Fla. Stat. § 456.47, which lets a physician evaluate you by video without requiring an in-person exam first.
Dr. Miranda is licensed in five states. If you move, split the year, or your family is spread across them, you keep the same doctor: Georgia, Ohio, Tennessee, Washington.
